Catchment Area

To join Weaverham surgery as a patient you are likely to have recently moved into the practice area. Please enquire at reception to obtain the required forms. The Practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, disability, medical condition nor appearance. From April 2015 all patients will be allocated a named, accountable GP who is responsible for a patients overall care at the practice.

You can register with us by using the online registration service

Initially we will ask you to complete a health questionnaire as it is important that we have as much information as possible on your past medical history. We will, of course, be asking the appropriate authority to send your medical records once they have been retrieved from your previous General Practitioner, but this will take at least 2-6 weeks. You will be offered a new patient health check with the practice nurse.

Named Accountable GP

We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone. Although, every patient has a “Named Accountable GP”.

You can ask for a particular GP to be your listed doctor and wherever possible we try to accommodate this.

Furthermore, we try to ensure you can have continuity of care. We do this by providing appointments wherever possible with the GP of your choice. But, there may be circumstances where this is not possible.
